●Appointments We see patients on an appointment basis. Please call for an appointment.We do provide Open Access for patients requesting same day appointments for routine and urgent care needs. All centers keep four time slots available for “Open Access” or “same day scheduling” for routine and urgent care based on patient preference or triage for each day. The morning session has availability of 10:00 AM and 11:30 AM time slots and 2:30 PM and 4:00PM time slots are kept open for afternoon session every day. All centers will try to accomodate patients at other time slots if the schedule is not booked or there is a "no show" of a booked patient.
●Payment for Services SWGHC provides access to health care for people, who might not otherwise be able to afford it. Health center services are not free. However, to ensure that income or lack of insurance is not a barrier to care, patients who are not covered by public or private insurance are charged on a sliding fee scale according to income. Co-payments are due at the time of the office visit. For patients who qualify for our sliding fee program, the patient’s percentage is due at the time of service. All our centers accept Visa, Master Card, Discover, Personal Checks (with identification) and Cash.
●Sliding Fee Program Patients who wish to participate in our sliding-fee-program are encouraged to ask for a sliding fee application form, which includes verification of income. In order for a patient to qualify for the sliding fee program, the information must be provided at the time of the visit. If the qualifying documentation is not provided, the patient will be responsible for 100% of the cost for services rendered.
●Labs We offer onsite lab services. All charges for labs must be paid in full before services are rendered.
●Medical Record Release If you need your medical records for any reason in future, you will need to sign a “Release of Information” in our office. Please allow one week to process the copying of your chart. There is a charge for this service.
●Patient Comments We strive to provide quality patient care and excellent customer service. There are suggestion boxes at all our offices for your comments and suggestions to improve our services. You are welcome to use Contact Us feature on this website to send your comments.
●After Hours Access All offices have a clinician on call for patients to receive clinical advice for urgent care. Please call the telephone number of the office and you will be directed to the clincian on call when the office is closed. This service is not meant for requesting prescription refills, scheduling and cancelling appointments or for any adminsitrative issue which can be taken care of by appropriate staff during regular office hours.
